| Zarząd Medicalgorithmics S.A. z siedzibą w Warszawie („Spółka”) informuje o zawarciu przez Spółkę w dniu 17 sierpnia 2023 r. umowy z Górnośląskim Centrum Medycznym im. prof. Leszka Gieca Śląskiego Uniwersytetu Medycznego w Katowicach („Zamawiający”) w związku z wyborem oferty Spółki jako najkorzystniejszej w postępowaniu o udzielenie zamówienia publicznego o numerze: DZ.3321.147.2023, o czym Spółka informowała raportem bieżącym nr 19/2023 z dnia 7 sierpnia 2023 r. („Umowa”). Przedmiotem Umowy jest dostawa urządzeń do wykonywania telemetrii ciągłej w ramach projektu Śląska Cyfrowa Platforma Medyczna eCareMed - telemedycyna w zakresie telekardiologii i teleneurologii w GCM: Śląska Cyfrowa Platforma Medyczna eCareMed - Telemedycyna i eksploracja danych medycznych (wraz z zadaniem pn. włączenie usług teleonkologii Uniwersyteckiego Centrum Klinicznego do Śląskiej Platformy Medycznej eCareMed oraz zadaniem pn. włączenie telekardiologii i teleneurologii Górnośląskiego Centrum Medyczne im. prof. Leszka Gieca do Śląskiej Cyfrowej Platformy Medycznej) Zad. 1 – Telemetria ciągła. Wartość wynagrodzenia określonego w Umowie za zrealizowanie całej Umowy wynosi: 1 589 925,00 zł netto, 1 789 857,75 zł brutto. Umowa w sprawie zamówienia publicznego została zawarta zgodnie ze wzorem stanowiącym zmieniony Załącznik nr 4 do Specyfikacji Warunków Zamówienia (SWZ), stanowiącym załącznik do niniejszego raportu. W sprawach nieuregulowanych w Umowie stosuje się przepisy Kodeksu Cywilnego i Ustawy Prawo zamówień publicznych z dnia 11 września 2019 r. (t.j. Dz. U. z 2022 r., poz. 1710 z późn. zm.). |

| Subject: Information on the conclusion by Medicalgorithmics S.A. with its registered office in Warsaw of a public procurement agreement for the supply of devices for continuous telemetry Current report no.: 20/2023 Date of preparation: 17 August 2023 Legal basis: Article 17(1) of the MAR Regulation – inside information. The Management Board of Medicalgorithmics S.A. with its registered office in Warsaw ("Company") informs that on August 17, 2023 the Company concluded an agreement with the Prof. Leszek Giec Upper Silesian Medical Center of the Medical University of Silesia in Katowice ("Contracting Authority") in connection with the selection of the Company's offer as the most advantageous in the public procurement procedure number: DZ.3321.147.2023, of which the Company informed in current report No. 19/2023 of August 7, 2023 ("Agreement"). The subject of the Agreement is the delivery of devices for continuous telemetry as part of the project Silesian Digital Medical Platform eCareMed - telemedicine in the field of telecardiology and teleneurology in GCM: Silesian Digital Medical Platform eCareMed - Telemedicine and medical data mining (along with the task entitled inclusion of teleoncology services of the University Clinical Center to the Silesian Medical Platform eCareMed and the task entitled inclusion of telecardiology and teleneurology of the Upper Silesian Prof. Leszek Giec Medical Center to the Silesian Digital Medical Platform) Ex. 1 – Continuous telemetry. The value of the remuneration specified in the Agreement for the performance of the entire Agreement is PLN 1,589,925.00 net, PLN 1,789,857.75 gross. The public procurement Agreement was concluded in accordance with the template constituting amended Attachment No. 4 to the Terms of Contract (SWZ), constituting an attachment to this report. In matters not covered by the Agreement, the provisions of the Civil Code and the Public Procurement Law of 11 September 2019 (i.e., Journal of Laws of 2022, item 1710, as amended) shall apply.
